 Root Canal Treatment of Upper Right First Molar with 5 Canals


Case presentation:

This 57-year old male patient presented with an intermittent, dull, aching pain on right hand side of his face. He identified the UR6 which had recently had a crown placed and reported the tooth was very sensitive to heat. 

Diagnosis: 

Irreversible pulpitis associated with recently treated carious cavity.

Treatment:

  • Molar root canal therapy was completed over 2 visits.

  • Rubber dam isolation and thorough chemo-mechanical preparation was undertaken using Wave One primary, reciprocating nickel titanium rotary files, sodium hypochlorite and EDTA.

  • 5 root canals were found, with separate MB1, MB2 and distal canals. The palatal root had 2 canals, the second palatal canal converging with the main palatal canal at 16mm.

  • The canals were obturated with taper matched gutta percha cones, AH Plus sealer and warm vertical compaction.

  • The tooth was symptomless at 12-month review.
