The patient will be returned to your care with a provisional restoration, unless you specifically request that we provide the permanent coronal restoration.

I will not perform any other treatment without your expressed consent.

I do not allow referred patients to join any practice at which I am based.

I don’t pass negative judgement on previous endodontic attempts.
